Understanding Rosacea

Rosacea is a chronic, inflammatory, and progressive skin condition that typically impacts the central part of the face and the eyes. It causes the dilation of blood vessels, which can lead to vascular hyper-responsiveness and even breakage or permanent damage if left untreated.

This condition affects around 10% of the population, and it’s more common in people with lighter skin, middle-aged adults, and women—men can also get it and when they do it tends to be more severe. One of the characteristic signs of rosacea is persistent redness in the area of the cheeks, similar to a sunburn or blushing. However, it is not the only one, since redness can be accompanied by other symptoms. These signs tend to appear due to certain factors, known as triggers.

Some common rosacea triggers include:

  • Stress or embarrassment
  • Hot or cold temperatures
  • Eating spicy or hot foods
  • Drinking hot beverages
  • Certain exercises
  • Facial cleansing
  • Sun exposure

The 4 Stages of Rosacea and Their Symptoms

Because this is a chronic and progressive skin condition, it can advance through its 4 stages if left untreated. Although this condition is mainly characterized by redness in the central area of the face, there are other symptoms that can accompany it depending on the stage of the disease.

The 4 stages of rosacea are:

  1. Pre-Rosacea
  2. Mild Rosacea
  3. Moderate Rosacea
  4. Severe Rosacea

      Stage 1: Pre-Rosacea

During this first stage signs like redness in the face—similar to flushing—might appear. Some could think this reddening can be attributed to heat, sun exposure, strong emotions, or certain situations. But pay attention if you start noticing that the cause of that blushing is related to the triggers mentioned earlier, because when it comes to pre-rosacea, redness is often caused by specific factors and disappears shortly after the cause is over.

      Stage 2: Mild Rosacea

The second stage is where blood vessels start to be more visible and symptoms like redness linger for longer than before—even long after the trigger has disappeared—irritation and/or swelling may accompany the previous symptom. All of this resulting from blood vessels remaining open. This is also the stage where minor structural damage starts being visible, and where seeking rosacea treatment becomes necessary.

If you notice any of the above signs and symptoms, seek medical help. At CareLife, professionals like Dr. Fatima Aziz, MD, and Dr. Syed Ashraf, MD, FHM, and the rest of the team are here to help you gain control of the situation and prevent further damage.

      Stage 3: Moderate Rosacea

The third stage is usually where redness starts persisting for more than a few hours, lingering for days, weeks, or even looking as if it has permanently settled on the face. This redness can cover more than just the cheeks, appearing in the nose, the chin, or the forehead as well—which could also be described as looking sunburned or windburned.

Redness is not the only symptom; during this stage it is also possible that patients with rosacea experience more symptoms, such as:

      • Papules (solid, small, and elevated areas of skin inflammation)
      • Pustules (papules filled with pus)
      • Swelling and inflammation
      • Burning sensations
      • Irritation

During this stage, blood vessels start showing different levels of damage, from normal function to shifts in the way they work, and even permanent damage.

      Stage 4: Severe Rosacea

The fourth stage is where blood vessels become significantly damaged, with fluids and proteins leaking into surrounding tissues—producing changes in the shape, texture, and tone of the skin. This is also a difficult-to-treat stage where symptoms are more severe, going beyond long-term redness or mild irritation. Other signs that may accompany this stage include:

      • Thickened skin
      • Enlarged pores
      • Irregular skin texture
      • Severe inflammation
      • Prominent blood vessels
